My Personal Health Plan Tool 
Soon, HHC patients will be able to access their own health information just by logging on to a secure internet site. When the new tool – MyHHC.info – rolls out in the coming weeks, HHC patients will be able to see their visit summaries, lab results, care plan, and other personal health information at any time by signing into their own secure account. 

To access their health information, patients must register for a free My Personal Health Plan account by visiting MyHHC.info and using their e-mail address.

Here are three things you need to know about My Personal Health Plan.
MyHHC.info is the gateway to My Personal Health Plan, a simple, powerful, no-cost tool to help our patients manage their health online. It enables patients to securely view and track their own health information anywhere, anytime. Patients with care plans can also use it to send messages to their provider.

1. My Personal Health Plan is a major benefit for our patients.

Research shows that patients pay more attention and become more engaged in their own healthcare when they have easy access to their health information.

My Personal Health Plan does just that – it enables patients to stay involved in their own care. It makes it simple for patients to securely view their care plans, discharge documents,
diagnoses, and more, all in one place.

2. My Personal Health Plan is great for providers, too.

Patients already rely on technology to communicate and are looking to their providers to do the same. Clinicians using tools such as My Personal Health Plan at other health systems say it makes communicating with patients easier, quicker, and more efficient. For example, such tools have been found to reduce phone call volume for non-urgent care issues by enabling patients to send secure text-based messages to their providers.
